WellFort Community Health Services – Four Corners Health Centre

WellFort is a not-for-profit organization providing health promotion, primary health care, oral health care, diabetes, and HIV and Hep C education in Peel Region. Four Corners is a community health centre and a member of the WellFort family, located in Malton. Four Corners works towards building a healthier Malton by providing primary care, community programs, health promotion and diabetes education, and by being reflective and responsive to the diverse needs of the community. Our programs and services are geared towards Malton residents who experience barriers to good health, including families who are under resourced; isolated seniors; newcomers; members of racialized communities and youth. Our innovative services are delivered through the use of interdisciplinary/ inter-professional teams, including health promoter/ educators, physicians, nurse practitioner, nurses, chiropodist, physiotherapist, social worker and diabetes educators.
